What services may be covered by my insurance plan?
Pure Wellness is in network with most major insurance plans and we are happy to verify that you have coverage for our Pain Management and/or Vein Services. Our staff will get any precertification that your insurance carrier may require.
If a service is covered by insurance, do I need a referral?
We do not require a referral for us to evaluate your medical problem; however, some insurance plans, such as HMOs require a referral from your primary care physician before your appointment.
Do you offer hair removal for tanned and dark skin?
Yes, we use the ND:YAG laser which is the only laser safe for tanned and all African-American skin types.
How many treatments will I need for hair removal?
Typically most people need 4-5 treatments for 80-90% permanent hair removal spaced one month apart.
What can I expect on my first Pain Management visit?
Your initial evaluation for pain chronic pain management will include a complete medical history, symptom focused physical examination, and review of any relevant radiological studies. Dr. Girault will recommend a comprehensive treatment plan which may include referral to alternative or complimentary therapies such as acupuncture, massage, or physical therapy.
What can I expect on my initial evaluation for varicose veins?
You will receive a complete medical history, physical examination and an ultrasound evaluation of your leg veins. Dr. Girault will recommend a treatment plan to improve your symptoms from your varicose veins.
What forms of payment do you accept?
We accept VISA, MasterCard, check, and cash. We also offer a patient financing program through Care Credit for procedures or treatments not covered by insurance. You may apply in our office for 30, 60, or 90 day payment plans.
